Basics and Pitfalls in TMJ Arthroscopy

  In contemporary concepts, the goals of temporomandibular joints surgeries are preservation of synovial membrane and fluid, TMJ arthrocentesis and arthroscopic surgery (lavage, lysis, and manipulation) are popular and used widely. TMJ arthroscope is powerful tools to accomplish the goal of preservation of TMJ tissue, there are many indications and advantages: from TMJ internal derangement to myofascial pain syndromes are indicated for arthroscopic surgery, short time consuming surgery and admission, tiny scar formation and after-surgery pain. At the same time, TMJ arthroscope could give surgeon the real time diagnosis of the patients' TMJ, like perforation, adhesion, chondromalacia, these diagnosis could not be made in any other diagnostic tool. The contraindications of the TMJ arthroscope are ankylosis, infection of TMJ, malignant tumor etc., we surgeons should keep in mind. Finally the complications most occurred of TMJ arthroscopic surgery are otologic complications: external auditory canal laceration, blood clotting in external auditory canal, ear fullness, vertigo, even partial hearing loss, the second often occurred complication is facial nerve injury, temporary or permanent. In this speech, we will share the experiences in the TMJ arthroscopic treatments, from LLM (lysis, lavage, and manipulation) to arthroscopic plication.

The Pathogenesis and the Potential Medical Treatment of Ameloblastoma

Ameloblastoma is the most common benign odontogenic neoplasm of dental epithelial origin in jaw bone but with aggressive behavior and a high recurrence rate. Nowadays wide surgical resection of the jaw bone is the only treatment available and recommended, which can cause further loss of craniofacial complex function and esthetics. The knowledge of pathogenesis of ameloblastoma remains limited until recent high frequency of BRAF and SMO mutations identified in ameloblastomas, especially FGFR2-RAS-BRAF-MAPK pathway has been identified as the most frequent mutation occurring pathway in ameloblastomas. Among all mutations, BRAF(V600E) mutation is the most common mutation in ameloblastomas. Since these mutations have been identified in other tumors and targeted therapies for these mutations are available. As the first step of using targeted therapy for treating ameloblastoma in the future, we have studied the frequency of common mutations in ameloblastomas in Taiwan, have determined the reliable detection method for BRAF(V600E) mutation in ameloblastomas and have investigated the cellular effects of BRAF(V600E) to dental epithelial cells and the potential drug resistance problem.
